In a 19th-century Europe, an idealistic physician, Dr. Robert Carroll, finds himself ensnared in a chilling intrigue when he receives a strange call to attend to a mysterious nobleman named Sardonicus. This aristocrat, who lives in isolation in his imposing castle, harbors a dark secret behind a face disfigured by a curse, which grants him a disturbing and frightening laugh. As Dr. Carroll delves into the life of the enigmatic noble, he discovers that Sardonicus is desperately seeking a remedy for his horrific condition, which is tied to a painful past and a failed experiment related to resurrection.

As the doctor tries to help him, Sardonicus reveals his twisted and manipulative nature. The situation quickly escalates into a battle of wills between the two men. The aristocrat is willing to do whatever it takes to get what he desires, involving the physician's wife, who also becomes a pawn in Sardonicus's dark games. As the terror intensifies, Dr. Carroll plunges into a web of psychological horror that challenges his ethics and tests his limits. With unexpected twists and a growing atmosphere of tension, the confrontation culminates in a fierce struggle between love, despair, and terror, leaving the viewer questioning the true cost of human ambition.
