A famous mystery novelist, Paul Sheldon, suffers a terrible car accident in a remote mountainous region. When he wakes up, he finds himself under the care of Annie Wilkes, his "number one fan," who just happens to be a retired nurse. At first, Paul is grateful for the attention he receives, but he soon discovers that Annie is an unstable and obsessive woman who not only knows every detail of his work but also has a dark secret: she does not like the direction his favorite character, Misery, has taken, and she is willing to do whatever it takes to "correct" it.
Isolated and with limited mobility due to his injuries, Paul is forced to appease Annie, who insists that he rewrite the story of Misery.
