In an extravagant and disturbing private prison in the desert of the United States, the director of the facility, Bill Boss, is a cruel and ruthless man who has no limits when it comes to maintaining control. His tyrannical approach ultimately draws attention from both the public and the authorities. With a twisted plan in mind, Boss decides to take torture and horror to a new level: to create the most chilling masterpiece ever seen.
As he faces pressure from the media and disrespect from the inmates, Boss contemplates creating his own version of a famous "work of art" in which a group of prisoners is united in the most grotesque way imaginable. With the help of his right-hand man, a mad surgeon, the director sets his twisted idea in motion: to form a “human centipede” composed of the inmates. The premise becomes increasingly absurd and terrifying, challenging the limits of morbidity and imagination.
In this horrifying spectacle, chaos ensues, and the prisoners find themselves caught in a spiral of madness and suffering. Amidst this frenzy, an internal conflict arises as some inmates begin to question the morality of their actions. With a mix of horror and dark humor, the film takes the viewer on a hallucinatory journey through the darkest corners of the human mind and what can emerge when cruelty becomes entertainment.
