In a small town in Arkansas, a man named Karl Childers, who has been locked away in a psychiatric hospital since his adolescence for a brutal murder, is released after decades of treatment. Appearing seemingly simple and with a peculiar way of speaking, Karl carries a dark past and a unique perspective on life. Upon arriving in the town, he begins to explore his newfound freedom, but adapting to everyday life is not easy.

Karl ends up helping a young boy named Frankie, who is abused by his mother and her abusive boyfriend. As he forges a heartfelt relationship with Frankie, Karl finally feels he is in a place where he can be accepted and loved, something he has longed for years. However, the darkness of his past continues to follow him, and as the boyfriend's violence escalates, Karl confronts his most primal instincts and his desires to protect the innocent.

The movie delves into themes such as redemption, loneliness, and the internal struggle of humanity between good and evil. Through his distinctive language and sincere outlook on the world, Karl becomes a symbol of how love and violence can intertwine in unexpected ways.
